Does the 2012-13 Disney Dining Plan still include children getting an appetizer? I am reading conflicting answers. Some are saying the kids still get an appetizer, beverage, entree, and dessert while others are saying that they have eliminated it.

I think there are conflicting answers because it's dependent on the restaurant.
Children ages 3-9 must order from the kid's menu when available. At Cinderella's Royal Table, the meal includes a choice of appetizer, entree, dessert, and beverage.
At counter-service restaurants such as Columbia Harbour House in the Magic Kingdom, kids get
an entree plus grapes, Dannon Danimals Yogurt, and a beverage. There is still the option to substitute items such as applesauce, cookie, and french fries. I recently ate here and had a kids meal for lunch. I had nuggets, fries, beverage, and a cookie.
Therefore if the child's menu includes the choice of an appetizer, then it's included on the Disney Dining Plan.
